ACKNOWLEDGEMENT OF PSYCHIATRIC POLICY

Notice of Psychiatric Policy

This document contains policies held by the psychiatric team in addition to outlining ways to ensure success with psychiatric medication services. These policies are in addition to Options’ general Acknowledgement of Policy.

 

Appointments

  • Arrive 15-20 minutes early to allow time for paperwork, vital signs, and to provide recent health updates.
  • Bring a list of all medications, vitamins, and supplements to every appointment.
  • Clients should tell their provider about substances they have used or are currently using.
  • The psychiatric team recommends that clients take notes during their appointment and throughout the month to ensure concerns can be discussed.
  • Schedule a follow-up appointment before leaving the clinic, and copy the appointment time and date on a card, in a phone, or in a planner.

 

Between Appointments

  • Follow the treatment plan and instructions on prescribed medication(s).
  • If a client has questions or concerns about their medication, treatment plan, side effects, or experience a change in symptoms, they should call the office to speak with someone on the psychiatric team.
  • For safety, the prescriber will generally want to see the client before making medication changes and may require collection of vital signs.

 

Regarding Medications

Psychiatric prescribers prescribe medications to help treat mental health conditions and they only prescribe psychiatric medications. The client’s primary care provider is responsible for other medications including, but not limited to, medications for blood pressure, diabetes, seizures, primary insomnia, and pain management.

 

Options psychiatric providers do not start new prescriptions for benzodiazepines and sedatives and they do not continue prescriptions of them for long-term use. If a client has a current prescription for any benzodiazepine or sedative medication, Options will verify the prescription with the current provider and will then work to taper off the medication in favor of safer treatments. Examples of benzodiazepines are Clonazepam/Klonopin, Lorazepam/Ativan, Diazepam/Valium, and Alprazolam/Xanax.

 

Options psychiatric team does not prescribe medications for detox from opioid pain medications, heroin, or alcohol. This includes, but is not limited to, benzodiazepines, suboxone, and methadone. Clients are recommended to enter in-patient, medically supported detox treatment for assistance with withdrawing from these substances.

 

Clients with a diagnosis of ADD/ADHD with current or recent untreated substance use disorders will not be prescribed psychostimulant medications. These include Adderall, Amphetamine salts, Ritalin, Methylphenidate, and other stimulants or controlled substances.

 

Options psychiatric providers do not prescribe medical marijuana.

 

Safe Prescribing Practices

Vital signs including temperature, pulse rate, respiration rate, blood pressure, and body weight are required to be collected on an annual basis and may be required more often at the discretion of the provider. Medication refills may be denied if vital signs have not been collected in the last year or at the time required by the prescriber.

 

Bloodwork is periodically required, at a schedule set by the provider, for anyone taking medications. The Options provider may require bloodwork be completed prior to scheduling a follow-up appointment. Bloodwork is usually required first thing in the morning after fasting for 10-12 hours (no food, clear liquids only).

 

Medication Refills

Clients are responsible for refilling their medication(s) in a timely manner.

  • Alert the pharmacy five (5) days before a refill is needed. It is recommended to speak to someone at the pharmacy, in person or over the phone, to ensure the refill request is accurate.
  • Each time a prescription is refilled check how many refills are available. Contact the psychiatric team if a refill is needed before the next scheduled appointment.
  • Ensure appointments are scheduled with the provider at the time they recommend. Some medications require frequent visits to ensure safe, ongoing refills.
  • Prescribers may be unable to refill prescriptions if an appointment is missed, based on individual needs and safety practices. Missed appointments should be rescheduled as soon as possible.
  • Refills provided between appointments may only contain enough medication to last until the next scheduled appointment.

 

Additional Treatment Services

Participation in therapy, skills training, and/or case management is required for all clients engaged in psychiatric medication services unless the Licensed Medical Provider indicates otherwise.

 

Missed Appointment Policy

In order to meet treatment goals, it is important to attend all scheduled sessions. Occasionally it is difficult to attend a scheduled appointment. When that happens, clients are expected to call to reschedule 48 hours or more before the appointment time. At Options, a missed appointment is defined as either canceling or rescheduling an appointment with less than 48 hours’ notice or failing to attend a scheduled appointment without any notification.

 

Missing two consecutive appointments or three appointments within a six-month period will result in discharge from psychiatric services. A missed appointment is defined as:

  • Canceling or rescheduling an appointment with less than 48 hours of notice
  • Failing to attend a scheduled appointment without notification
